A 70 yr old male† presented to Rural Health Training Centre (RHTC), Mendhasala, under the department of Community & Family Medicine, All India Institute of Medical Sciences, Bhubaneswar, India, in October 2019, with a 34 yr history of swelling of the right upper limb (Fig. 1) and left lower limb (Fig. 2). On examination, it was found that the oedema was irreversible, non-tender, non-pitting with the presence of knobs and non-healing ulcers as the patient was also diabetic. According to the Dreyer system of classification, his lymphoedema was classified as grade 4, diagnosed as lymphatic filariasis. Training - home-based self-care management including simple hygiene measures, skin care to prevent dermatolymphangioadenitis were imparted. Follow up visits of four weeks interval were pursued to ensure the self-care practice.

- Upper right limb lymphoedema.

- Right upper and left lower lymphoedema.
