A 64 yr old male patient of abdominal aortic aneurysm was referred to the department of Cardiovascular Radiology and Endovascular Interventions, All India Institute of Medical Sciences, New Delhi, India, with contained rupture in October 2019, which was successfully managed with endovascular stent graft (Figure A-E). Follow up computed tomography (CT) after nine months showed patent aortic stent graft in situ with no leak (Figure F).

Endovascular aortic repair in abdominal aortic aneurysm with contained rupture. (A) Volume-rendered reconstruction, (B) coronal, (C) axial computed tomography image showing infrarenal abdominal aortic aneurysm with contained rupture (white arrow). (D) Digital subtraction angiogram revealing infrarenal abdominal aortic aneurysm. (E) Abdominal aortic stent graft with iliac extension device was deployed excluding the aneurysm with no contrast leak. (F) Follow up computed tomography showing aortic stent graft in situ with no leak.

Ruptured abdominal aortic rupture needs emergency treatment either by surgery or endovascular aortic repair (EVAR). The immediate and acute outcomes of EVAR with Y-shaped stent graft are superior to surgical repair in patients with suitable anatomy. Recent advances in the fast acquisition CT with multiplanar reconstructions can enable in immediate treatment planning in such patients. Endovascular aortic stent graft is an attractive option in such emergency conditions, with the advantage of being less invasive and less morbid.
